Waiver and Release
Healing Wisdom USA Corp.

In consideration of Healing Wisdom USA Corp. (“Healing Wisdom”) furnishing services and/or equipment and permitting me to participate in its classes, workshops, seminars, trainings, retreats, events, and other activities (“Activities”), I agree, on behalf of myself and my successors, assigns, heirs and personal representatives, as follows:

I understand that I may leave the Activities at any time for any reason.

I fully understand and acknowledge that the Activities have inherent foreseeable and unforeseeable risks, my participation in Activities may result in injury or illness, and the risks of the Activities may arise from foreseeable or unforeseeable causes including the negligence of others, accidents, diseases, and other causes. I also understand that the Activities may bring up issues of a highly personal nature that may cause me to experience emotional or physical responses that may be unexpected and/or unpleasant. Further, I understand that I may experience mental, emotional, physical, or spiritual distress and that such distress may cause unpleasant feelings. What is experienced in these Activities may create physical responses on my part or on the part of other participants.

As a condition of my participation in Activities, I represent and warrant that I am fully capable of participating in Activities, and that I have not and will not consume any alcoholic beverages or other substances that would impair my senses or abilities. I confirm that I do not suffer from any mental or physical impairment and have not been diagnosed at any point with a disorder, condition, or injury, either physical or mental, that might make it inadvisable for me to assume such risks.

I hereby accept and assume all risks of my participation in Activities, whether disclosed or undisclosed, foreseeable or unforeseeable, and accept and assume all responsibility for any liability, claims, demands, losses, expenses or damages whether caused in whole or in part by the negligence or other conduct of Healing Wisdom or any other person. I hereby release and discharge, waive any right, covenant not to sue, and agree to hold harmless, defend, and indemnify Healing Wisdom and its officers, directors, members, managers, agents, employees, successors and assigns, from any and all liability, claims, demands, losses, expenses or damages for or arising out of any bodily injury, disease, property damage, wrongful death, loss of services or otherwise which may arise out of my participation in Activities.

I acknowledge the Activities are not designed as a substitute for therapy or any other form of professional consultation. The Activities are designed as an educational venue only.

I further irrevocably agree and consent that Healing Wisdom and its successors and assigns may produce and reproduce audio and audiovisual recordings and still photographs (“Media”) of me and my participation and performance in Activities and to edit, alter, distribute, perform, display, exhibit and otherwise use the Media in advertising and promotional materials throughout the world in perpetuity in any media now known or hereafter developed. Healing Wisdom shall have all right, title and interest my likeness and performance in connection with the Media. This consent is given as an inducement for Healing Wisdom to produce Media and I understand Healing Wisdom will incur substantial expense in reliance thereof. Healing Wisdom is not obliged to make any use of Media or exercise any of the rights granted by this release. I hereby release Healing Wisdom and its officers, directors, members, managers, agents, employees, successors and assigns from all claims based on use of the Media. 

Notwithstanding the foregoing, Healing Wisdom will upon my request provide me with prior written notice in each instance of its intention to distribute Media which includes my likeness or performance.
